Our Purpose and Mission

Our mission at Patient Centric Solutions (PCS), is to enable patients and providers to easily exchange healthcare data so accurate clinical data is always available at the right place to inform care.  Our objectives are to eliminate gaps in care; improve outcomes; and reduce hassle, burden, and cost. We believe that collaborative patient care is a team sport, including many types of clinicians, the patient and family caregivers.

We are committed to safeguarding your privacy online. We do not collect personally identifiable information about any individual except with their permission. We adhere to the HIPAA privacy rule in protecting individual health records and other personal information. We do not disclose your private online communications with us to any third parties. We also don’t disclose your registration or healthcare data to any third party unless you’ve specifically authorized them to see your information. We respect any limits you set up about the data elements that people can see or the length of time they’re authorized to have access to your information.


Information We Collect

Our app, PatientShare, is a SMART on FHIR client. This means it uses a specific protocol to securely connect to health systems such as your provider’s Electronic Health Record (EHR). SMART stands for Substitutable Medical Applications, Reusable Technologies. FHIR stands for Fast Healthcare Interoperability Resources. Both are initiatives to simplify the exchange of healthcare data. A SMART on FHIR client is an interface to an electronic health information system, and does not have control over the underlying health information system or the health information system’s policies. Please refer to the health information system’s privacy policy to understand those policies. 

As a SMART on FHIR client, we do not collect or store any of your personal information unless that information is necessary to create an account to access specific capabilities or monitor application usage and performance.  Such information may include: a unique identifier or IP Address. No personal data is used or disclosed for any purposes without informed, proactive consent from the user. The application may collect metrics indicating site usage, but personally identifiable infomation is not used for such purposes.

We may disclose information about our users when we believe that the disclosure is required by applicable federal and state law.

PCS is concerned with the security of the personally identifiable information we collect at registration and the security of the data that we transmit on your behalf. We have in place measures to prevent unauthorized access to that information. PCS has developed a secure and reliable application using the best practices associated with the protection of personal data.

Communication and Services

If you have established an account for purposes stated above, and you no longer wish to have a PCS account, please contact us and PCS will help you close your account and fully delete your personal information.

PCS provides the information contained within the application to you, subject to the terms and conditions of use set out here. By using PCS services, you are deemed to have accepted and agreed to the terms and conditions. If you do not accept and agree to the terms and conditions, do not access, use or download any information with the application. PCS may modify the terms and conditions at any time. By using this application, you consent to the collection and use of the information described here. If we decide to make changes to this privacy policy, we will post the changes on this page, so that you will always know what information we collect and how we use it. By using the application subsequent to such changes, you consent to them.
