PatientShare in Production at Connected Care Exchange

Patient Centric Solutions (PCS) is excited to see our PatientShare solution in production at the Connected Care Exchange (CCE) Health Information Exchange, branded as CareShare. Implementing CareShare will reduce the physician burden when sharing or accessing crucial health data across facilities. It puts up-to-date, critical information in the hands of providers. That makes it easier for providers to offer informed, accurate, and effective healthcare that improves outcomes for their patients.
To achieve this solution, CCE combined Availity Fusion by Availity, the Aidbox FHIR API, and the PatientShare viewer to deliver a comprehensive patient record wherever it is needed to inform care.

CMS Connectathon …

Patient Centric Solutions participated in the 4th annual Centers for Medicare & Medicaid Services (CMS) and HL7 Connectathon. Patient Centric Solutions showcased our FHIR-first PatientShare solutions in PACIO (Post-Acute Care Interoperability) track in a live test setting, demonstrating the benefits of having complete clinical data available to inform care, demonstrating the exchange of CMS assessment data in a Post-Acute setting. The eLTSS (Long Term Services and Support) care plan was created and updated to demonstrate the benefits of exchanging a collaborative care plan. Both PatientShare and the eLTSS client are SMART on FHIR apps and can connect to any FHIR R4 API.

CARIN Alliance Digital Identity POC

It was a pleasure for the Patient Centric Solutions (PCS) team to participate in the Carin Alliance Healthcare Digital Identity Proof of Concept. This important work will guide the industry toward a more interoperable, equitable, privacy-centric, resilient, and secure federated digital identity ecosystem.
The PoC demonstrated it is possible for patients to be reliably identity proofed remotely and access their medical records from all of the places where they've received care, without necessitating separate logins. PCS also demonstrated the power of patient-mediated exchange in combination with federated digital identity.
